RESUMO

The effectiveness of at-home dental bleaching treatments depends on the time that bleaching products are in contact with the teeth surface and, consequently, on the adequate use of associated custom acetate trays. OBJECTIVE: This randomized single-blinded trial aimed to analyze if the daily usage time of these products influences the patient's compliance behavior when submitted to monitored at-home dental bleaching. Secondary outcomes were color change and tooth sensitivity. METHODOLOGY: Sixty-six volunteers were randomly distributed into three groups (n=22): patients were instructed to use the trays for 2 (G2), 4 (G4), and 8 (G8) hours daily. The daily dental bleaching compliance behavior was measured using a microsensor inserted into the trays. Subjective and objective color evaluation assessments were adopted at baseline (T0), one (T1), two (T2), and three weeks (T3) after the beginning of the bleaching treatment, as well as two weeks after the treatment (T4). Tooth sensitivity was analyzed using the VAS scale, ranging from T1 to T4. RESULTS: G2 showed a greater degree of cooperation than G8 and cooperation was inversely proportional to the recommended usage time. Significantly higher color change was observed in the upper arch for G8 when compared to G2 in subjective analysis, from T1 to T4. There were no statistical differences between the groups in objective analysis. CONCLUSION: Shorter recommended usage time of the bleaching product may improve the patient's compliance with at-home dental bleaching treatments. However, increased daily usage time may promote better subjective color change. Bleaching sensitivity was more significant in the first week for a longer time of use.

RESUMO

An increase in the use of Computer-Aided Design (CAD) and Computer-Aided Manufacturing (CAM) technologies challenges the conventional prosthetic fabrication procedures that are practical and centered on a digital workflow for the patient, especially for dental implants. Increasing workflow of digital restoration work, considering computer-used CAM for restoration technology systems and also fast/CAM for building restoration technology; fast/CAD, also known as abut-Base, has increased interest. Studies on adaptation of different restorative materials, on titanium (Ti)-base abutments, traction, and transformed cycling have become relevant. The objective of this work was to research, through literature studies, on restoration-type abutments. A total of 24 articles were found after searching the following terms in PubMed/Medline, Scopus, and Embase databases: "ti-base AND abutment." Twenty-one manuscripts selected from the inclusion and exclusion criteria. After an analysis of these articles, it was concluded that the Ti-base abutment and components from the same manufacturer as the Implant should be used preferably; milled monolithic crowns designed to adapt to the Ti-base the hybrid abutment-crown assembly does not affect torque maintenance after thermal aging; the saliva and cleaning protocol of the Ti-base bonding surfaces can influence the operations of the Ti-base crowns; Ti-base and Crown surface treatment is recommended for better applicability and stability results, and the superiority of resin-based cements compared with other types of cements.

RESUMO

Background: To verify if there is a correlation of excessive daytime sleepiness (EDS) with bruxism, depression, anxiety, stress, and sex in odontology students. Material and Methods: Four hundred dental students were selected. Students included in the study were those that were healthy, over 18 years old, and with similar weekly academic activities. Students excluded were those with temporomandibular disfunction, a high possibility of possessing obstructive sleep apnea and illnesses that cause EDS; along with smokers, users of illicit drugs, users of psychiatric medication, and those that abuse the consumption of alcohol. After the application of these criteria, 128 students were included in this study. EDS, bruxism, depression, anxiety, and stress were evaluated by the Epworth Sleepiness Scale, the Pintado et al. questionnaire, Beck Depression Inventory, Beck Anxiety Inventory, and the LIPP Adult Stress Symptom Inventory. Afterwards, the Spearman test (p< 0.05) was applied. Results: A high probability of EDS was present in 33.6% of the students. There was a positive correlation of EDS with females (Rank Correlation= 0.209; p= 0.018) and depression (Rank Correlation= 0.174; p= 0.049); between females and stress (Rank Correlation= 0.199; p= 0.024), and between females and anxiety (Rank Correlation= 0.178; p= 0.045). There was a positive correlation of bruxism with anxiety (Rank Correlation= 0.255; p= 0.004) and stress (Rank Correlation= 0.201; p= 0.023). There was no correlation of EDS with bruxism (p= 0.354), stress (p= 0.277), or anxiety (p= 0.114). There was no correlation of bruxism with females (p= 0.082) or depression (p= 0.362). Conclusions: A high probability of EDS was present in 33.6% of dentistry students, and there was a positive correlation of EDS with females and depression. Key words:Bruxism, depression, anxiety, stress, disorders of excessive somnolence, dentistry.

RESUMO

Background: To determine whether there is a correlation of bruxism (sleep, daytime, or both) with oral health-related quality of life and facial pain of muscular origin in dentate individuals. Material and Methods: Seventy-four dentate patients (complete dentition) were included in this study. These individuals had pain in the facial muscles due to temporomandibular disorder (TMD). Smokers; and those with obstructive sleep apnea, TMD of joint origin associated or not with pain, malocclusion, and cancer; and users of illicit drugs, psychiatric medications, and alcohol were excluded. Obstructive sleep apnea, bruxism (of sleep and/or daytime), facial muscle pain, and oral health-related quality of life were assessed by the following questionnaires: Berlin Questionnaire, Pintado et al. questionnaire, VAS (Visual Analog Scale) facial muscle pain questionnaire, and Oral Health Impact Profile - 14. Four groups were created: 1) no bruxism; 2) sleep bruxism; 3) daytime bruxism; and 4) sleep and daytime bruxism. Spearman's correlation test was applied to verify if there was a correlation between the collected data. P values less than 0.05 were considered statistically significant. Results: There was a positive correlation of daytime bruxism with mean pain in the last 3 months (P<0.05) and the worst pain experienced in the last 3 months (P<0.05). Conclusions: Bruxism (sleep, daytime, or both) showed a positive correlation with lower oral health-related quality of life (P<0.05). Key words:Bruxism, facial pain, temporomandibular joint disorders, surveys and questionnaires, health-related quality of life.

RESUMO

OBJECTIVE: The aim of this study was to compare the direct relining technique with the indirect relining technique in relation to quality of life, satisfaction with the relining, occlusal force, and halitosis of users of acrylic complete dentures. MATERIALS AND METHODS: Twenty bimaxillary edentulous individuals were selected. They had to use the same pair of complete dentures for a minimum of 1 year and a maximum of 5 years. The subjects were randomly divided in groups of direct relining and indirect relining of the inferior denture (n = 10). Both groups received the same silicone-based relining. The clinical tests verified the quality of life (Oral Health Impact Profile in edentulous individuals), the satisfaction with the relining, the occlusal force, and halitosis. The tests (halitosis and occlusal force) were performed initially (before the relining), immediately after the relining, and 30, 60, 90, and 180 days after the relining. The questionnaires (quality of life and satisfaction with the relining) were performed initially (before the relining), and 30, 60, 90, and 180 days after the relining. STATISTICAL ANALYSIS: Analysis of variance and the Tukey test were used (p < 0.05). RESULTS: There was no statistical difference comparing the two techniques in all the evaluations (p < 0.05). There was a significant statistical difference for the factor of time in all clinical tests for each relining technique (p < 0.05). The quality of life and satisfaction with the relining increased significantly 30 days after the relining when compared with the initial time point, for both techniques (p < 0.05). The occlusal force increased significantly after 90 and 180 days when compared with the initial time point, for both techniques (p < 0.05). Halitosis decreased significantly immediately after the relining when compared with the initial time point, for both techniques (p < 0.05). CONCLUSION: Independent of the relining technique used, there was an increase in the quality of life, satisfaction with the relining, and occlusal force, as well as a reduction in the level of halitosis. Both techniques generated similar results and therefore can be options in clinical practice.

RESUMO

ABSTRACT Objective: To investigate the tissue response and the biomineralization ability of CER prepared with epoxy resin or water compared to Mineral Trioxide Aggregate (MTA). Material and Methods: Polyethylene tubes containing materials or empty tubes for control were inserted into the subcutaneous tissues of 30 rats. After 7, 15, 30, 60, and 90 days, the rats were killed and the tubes were removed for analysis using hematoxylin-eosin staining, von Kossa staining, and under polarized light. Inflammation was graded through a score system; the thickness of the fibrous capsule was classified as thin or thick; the biomineralization ability was recorded as present or absent. The results were statistically analyzed using the Kruskal-Wallis test (p<0.05). Results: Histologic analysis performed after 7 and 15 days for CER prepared with epoxy resin or water and for MTA showed moderate inflammation and a thick fibrous capsule (p>0.05). After 30, 60, and 90 days, mild inflammation, and a thin fibrous capsule were observed in all groups (p>0.05). Conclusion: All materials had structures positive for von Kossa and birefringent to polarized light. CER epoxy resin showed biocompatibility and biomineralization similar to CER water and MTA.

RESUMO

OBJECTIVE: The aim of this study is to evaluate the color alteration and shore A hardness of a medical silicone with extrinsic pigmentation, before and after accelerated aging. MATERIALS AND METHODS: Twenty samples (Silastic Q7-4735) were made with an intrinsic pigmentation. This intrinsic pigmentation was composed of a pink pigment (H-109-P, Factor II) and an opacifier (ZnO). All samples had standardized dimensions (45-mm diameter and 2-mm thickness). Half of the 20 samples manufactured subsequently received an extrinsic pigment (Tan FE-215, Factor II). Therefore, two groups were created (n = 10): Group 1, group with intrinsic pigmentation and without extrinsic pigmentation (control) and Group 2, group with intrinsic and extrinsic pigmentation. Samples were submitted to color and Shore A hardness tests, before and after 1,008 hours of aging. STATISTICAL ANALYSIS: Color alteration data were submitted to Student's t-test (α = 0.05). Shore A hardness data were submitted to two-way analysis of variance and Tukey test (α = 0.05). RESULTS: The incorporation of the extrinsic pigment on the silicone did not affect its color (ΔE) when the two groups were compared (p = 0.232). Regarding the hardness test, the interaction between group and period did not interfere with the hardness results(p=0.599). However, the period factor showed that there was a reduction in the hardness of the silicone after aging (p < 0.05). CONCLUSION: In this study, all the hardness and color results of the silicone used were clinically acceptable, regardless of the presence of extrinsic pigmentation.

RESUMO

This clinical report describes the enamel microabrasion technique for removing maxillary and mandibular hard fluorotic enamel stains followed by home-monitored home dental bleaching. The removal of fluorotic enamel stains utilized macroabrasion with a water-cooled, fine-tapered 3195 FF diamond bur followed microabrasion with the application of Prema Compound (Premier Dental Products Co, Norristown, PA, USA). Home-monitored dental bleaching was performed 14 days after enamel microabrasion using a 10% carbamide peroxide gel for 2 h/day. The wearing time of the acetate tray/dental bleaching was quantified by a microsensor from TheraMon microelectronic system (Sales Agency Gschladt, Hargelsberg, Austria) that was completely embedded in the acetate trays. The teeth were bleached effectively during 23 days. The mean wearing time of the acetate trays/dental bleaching product was 1.54 h/day, for the upper and lower arches. The patient reported satisfaction with the treatment. The association of enamel microabrasion and home dental bleaching was an excellent clinical treatment for teeth affected with enamel fluorosis.

RESUMO

OBJECTIVE: To analyze the influence of three pigment incorporation methods on color change, dimensional stability, and detail reproduction of the MDX4-4210 and A-2186 silicones. MATERIALS AND METHODS: The A-2186 and MDX4-4210 silicones were used for preparation of samples, with the incorporation of bronze, black and pink pigments, usingconventional, mechanical, and industrial incorporation methods. Samples were submitted to the initial readings of color (n = 10; 22-mm diameter × 2-mm thickness), detail reproduction, and dimensional stability (n = 10; 30-mm diameter × 3-mm thickness). Readings were also taken at the end of 252, 504 and 1,008 hours of aging cycles. RESULTS: Quantitative data were evaluated by ANOVA and Tukey test, with a level of significance of 5%. The mechanical and industrial methods caused smaller color changes of all samples compared with the conventional method (p < 0.05). In most cases, the mechanical and industrial methods caused less samples' contraction than the conventional method after aging (p < 0.05). The color change values increased progressively in each aging period for all samples (p < 0.05). The contraction values increased progressively in each aging period for all samples (p < 0.05). In the qualitative analysis of detail reproduction, all samples presented full reproduction of the three grooves, with accurate angles, initially and after the aging periods. CONCLUSIONS: The industrial and mechanical methods showed the best results for color and dimensional stability. Despite the statistical differences, all pigment incorporation methods generated acceptable dimensional and color changes of the MDX4-4210 and A-2186 silicones, regardless of the pigment and aging. In addition, the detail reproduction was satisfactory after aging periods in all cases of this study, showing the excellent quality of the A-2186 and MDX4-4210 silicones.

RESUMO

OBJECTIVES: The present dental bleaching case report describes a new method that precisely quantifies the daily wearing-times of the bleaching product by inserting a microsensor in the acetate custom tray. The bleaching efficacy was also discussed since the patient was previously submitted to enamel microabrasion. METHODS: The patient was submitted to enamel microabrasion in 1987, and bleaching treatment was performed in 2005. In 2017, re-bleaching was executed using 10% peroxide carbamide. The electronic microsensor, TheraMon (TheraMon® microelectronic system; Sales Agency Gschladt, Hargelsberg, Austria), was embedded in the labial region of the upper and lower acetate trays to evaluate the wearing-times of the acetate trays/bleaching product. The patient was instructed to wear the tray for 6 to 8 h/day while sleeping. After 24 days of bleaching treatment, the data obtained from the TheraMon electronic devices was collected and interpreted. RESULTS: The patient did not entirely follow the bleaching treatment as recommended, as there was no evidence of use of the upper and lower trays for some days; additionally, the bleaching product was used for shorter and longer periods than was instructed. CONCLUSIONS: The TheraMon microeletronic device precisely measured the wearing-times of the acetate tray/bleaching product during the bleaching treatment. Teeth submitted to enamel microabrasion presented with a healthy clinical appearance after 30 years. CLINICAL SIGNIFICANCE: Measuring the length and frequency of use of an acetate tray/bleaching product can be important to clinicians and patients for obtaining a controlled and adequate bleaching treatment.

RESUMO

The aim of the present investigation was to evaluate the craniofacial features of subjects with normal occlusion with different vertical patterns in the mixed dentition using morphometric analysis (Thin-Plate Spline analysis - TPS) applied to posteroanterior (PA) films. The sample comprised 39 individuals (18 females and 21 males), all in mixed dentition, aged from 8.4 to 10 years with satisfactory occlusion and balanced profile and with no history of orthodontic or facial orthopedic treatment. The sample was divided into three groups (mesofacial, brachyfacial and dolichofacial) according to the facial types proposed by Ricketts (1989). The average craniofacial configurations of each study group were obtained by orthogonal superimposition of Procrustes, thereby eliminating size differences and allowing only shape differences between groups to be analyzed by viewing the TPS deformation grid. Significant differences were found among the three facial types but were more remarkable between mesofacials and dolichofacials than between mesofacials and brachyfacials. TPS morphometric analysis proved efficient for accurate visualization of transverse and vertical differences among facial types even before pubertal growth spurt. These differences cannot be easily detected by traditional posteroanterior cephalometry.

The anterior open bite (AOB) can be defined as the presence of an existing ‘no vertical overlap’ of incisors edges of the upper, lower and front teeth. It is a malocclusion that affects both the male and the female, especially in childhood and during the changing of teeth. The aim of this study was to perform a narrative review of the deleterious oral habits that cause AOB. To carry out this research we used PUBMED, SciELO and LILACS databases, plus bibliographic presented in the library of Universidade Camilo Castelo Branco - Campus Fernandópolis / SP. Using the keywords Open bite; Etiology and Habits, from 2004 to 2015. It is important for orthodontists get to know the deleterious oral habits, since it is known that any previous interventions to prevent them result in improved quality of life through the establishment of appropriate supply conditions, breathing and speaking, that support harmony and the balance of skeleton, soft tissues, morphology and dental volume, which have a direct interference in occlusion. It concluded that this type of malocclusion when diagnosed and early intercepted increases the likelihood

All patients were submitted to rapid palatal expansion and dentoalveolar expansion of the lower dental arch with a lip bumper. Pre-treatment and post-treatment cast models were obtained and digitized with the 3Shape R700 3D (3Shape A/S, Copenhagen, Denmark) scanner. Transverse distances between the cusp tips of the canines, the mesial buccal cusps of the deciduous molars and of the permanent first molars and between the cervical middle points of the buccal aspects of these teeth were measured. The t tests were performed to determine whether an increase in transverse distances was signifi cant. Results: a statistically significant transverse increase (p < 0,05) ranging from 4.11 mm to 4.51 mm was observed in the cusp tips. Conclusion: the fixed and expanded lip bumper presented a significant transverse effect within approximately 6 months of treatment...

RESUMO

OBJECTIVE: To evaluate the long-term effects of the standard (Class II) Balters bionator in growing patients with Class II malocclusion with mandibular retrusion by using morphometrics (thin-plate spline [TPS] analysis). MATERIALS AND METHODS: Twenty-three Class II patients (8 male, 15 female) were treated consecutively with the Balters bionator (bionator group). The sample was evaluated at T0, start of treatment; T1, end of bionator therapy; and T2, long-term observation (including fixed appliances). Mean age at the start of treatment was 10 years 2 months (T0); at posttreatment, 12 years 3 months (T1); and at long-term follow-up, 18 years 2 months (T2). The control group consisted of 22 subjects (11 male, 11 female) with untreated Class II malocclusion. Lateral cephalograms were analyzed at the three time points for all groups. TPS analysis evaluated statistical differences (permutation tests) in the craniofacial shape and size between the bionator and control groups. RESULTS: TPS analysis showed that treatment with the bionator is able to produce favorable mandibular shape changes (forward and downward displacement) that contribute significantly to the correction of the Class II dentoskeletal imbalance. These results are maintained at a long-term observation after completion of growth. The control group showed no statistically significant differences in the correction of Class II malocclusion. CONCLUSIONS: This study suggests that bionator treatment of Class II malocclusion produces favorable results over the long term with a combination of skeletal and dentoalveolar shape changes.

RESUMO

OBJECTIVE: The purpose of this study was to assess by means of scanning electron microscopy (SEM) the remaining adhesive interface after debonding orthodontic attachments bonded to bovine teeth with the use of hydrophilic and hydrophobic primers under different dental substrate moisture conditions. MATERIAL AND METHODS: Twenty mandibular incisors were divided into four groups (n = 5). In Group I, bracket bonding was performed with Transbond MIP hydrophilic primer and Transbond XT adhesive paste applied to moist substrate, and in Group II a bonding system comprising Transbond XT hydrophobic primer and adhesive paste was applied to moist substrate. Brackets were bonded to the specimens in Groups III and IV using the same adhesive systems, but on dry dental enamel. The images were qualitatively assessed by SEM. RESULTS: The absence of moisture in etched enamel enabled better interaction between bonding materials and the adamantine structure. The hydrophobic primer achieved the worst micromechanical interlocking results when applied to a moist dental structure, whereas the hydrophilic system proved versatile, yielding acceptable results in moist conditions and excellent interaction in the absence of contamination. CONCLUSIONS: The authors assert that the best condition for the application of primers to dental enamel occurs in the absence of moisture.
